Keluarga Berimun is a 12-month childhood basic immunization demand-generation programme in Depok City and Bekasi Regency, combining digital communication and offline community engagement to address caregivers’ behavioural and practical barriers to immunization.

Over twelve months the campaign was seen 121.5 million times and reached 64.3 million people at IDR 26 (USD 0.0015) per person. The sharper engagement came from the two-way communication channels: 3,286 caregivers used the WhatsApp Hotline, 1,820 joined the Support Group, and members generated 21,942 organic messages of their own. Almost half of the caregivers accessed the channels through a community health worker's WhatsApp message (49.5%) and another 43.8% through a referral at a Posyandu visit, which suggests the digital platforms borrowed their credibility from existing ‘relationships’ and offline community engagement.

The independent impact evaluation, comparing Depok and Bekasi against control areas in Semarang City and Kendal Regency, found caregivers in intervention areas were 14.5% more likely to believe immunization protects against disease, 12.3% more likely to consider it important for their child's health, and 9.1% more likely to trust a health worker's recommendation. The key insight from this research is that caregivers reached while pregnant were 17 percentage points more likely to have a child who completed the full immunization schedule. The cost-effectiveness analysis data also shows similar findings where moving caregivers from knowledge to action required greater investment, especially after missed doses.

Caregiver reached on early phase costs IDR 82,307 per person while recovering one who has already missed a dose costs IDR 1,036,945
