Indonesia’s fragmented health data systems have long posed challenges for maternal and child care. Pregnant women and parents of young children often face barriers to reporting their health status, while community health workers (CHWs) and primary care professionals struggle to track outcomes and follow up in time, particularly in underserved areas. CISDI developed the PN PRIMA App as a trusted bridge connecting CHWs and health professionals with the mothers and children they serve.


Built with a Human-Centered Design approach and continuous feedback from CHWs, the app integrates critical data, from nutritional status to medical history, into a single platform that CHWs and primary care professionals can both reach. Ongoing training and support build digital confidence among CHWs, so the app works as a bridge for better health communication rather than one more system to maintain.


This two-pager sets out how the app works, from pulling target data out of the Puskesmas ePPGBM system, through nutrition data confirmation by health workers, to CHW home visits and intervention and the return of processed data to the Puskesmas. It closes with what its use has produced: higher CHW satisfaction, a high retention rate among users, and significant uptake. The app has been implemented in three districts in West Java: Bekasi Regency, Depok City, and Bandung City.
